Personalised Communication Preferences Card

A personalised communication preferences card lets you choose the words that describe what helps you. Unlike a stock message card, this original ACB design provides space for your own main request, a supporting line and three preferences on the reverse. The blue-violet heading and dark text keep the focus on the information you want another person to read.

Personalised communication preferences card details

Replace the main message and supporting line on the front with your own concise wording. On the reverse, add up to three short preferences. You might ask for written information, extra processing time or a particular way of asking questions, if those requests suit you. Edit or remove anything that does not express your own choice.

Choosing the information to print

The person using the card should choose or agree the messages wherever possible. Read both sides together and check that the wording remains useful in the situations where it may be shown. A name, photograph, diagnosis or medical history is not required by this design. Share only the information that you want readers to see.

What is included?

You receive one double-sided plastic wallet-sized card, approximately 85 x 54mm, printed with the details you approve. A holder, lanyard, clip, writable panel, Braille and raised symbols are not included. Longer text may require a smaller font or revised layout, so keep the message concise and check its legibility at actual card size.

Check spelling, telephone numbers, layout and the final front and reverse proof before approving production.
