Personalised Carer Emergency Contact Card

A carer emergency contact card lets you carry a concise message that you care for someone, alongside the details of a person you choose to contact. This original ACB design keeps the front focused on that message and puts the contact information on the reverse. It is a personalised printed card rather than membership of an emergency-care scheme.

Carer emergency contact card details

Add your own name to the front. On the reverse, supply the chosen contact's name and telephone number, then edit or remove the relationship line. Use details that the contact has agreed you may print and carry. The wording asks someone to call that person if you cannot contact the person you care for.

Choosing the information to print

Agree your arrangements with your chosen contact before using the card. Check the number digit by digit and keep the card up to date when those details change. Avoid including the cared-for person's address, medical history or other private information unless you have a clear reason and appropriate permission. The card does not store or activate a care plan.

What is included?

You receive one double-sided plastic wallet-sized card, approximately 85 x 54mm, printed with the details you approve. A holder, lanyard, clip, writable panel, Braille and raised symbols are not included. Longer text may require a smaller font or revised layout, so keep the message concise and check its legibility at actual card size.

This carer emergency contact card does not provide a monitored telephone line, replacement care, registration, official carer status or guaranteed assistance.
